Medical Navigation is a service that patients and families can engage. It can be defined through Patient advocacy and incorporates clinical knowledge, mediation/facilitation skills and an in-depth understanding of the healthcare system and process.
We will come into the acute setting at your request and with your permission review clinical records and offer third party assistance through Health and illness education, anticipatory guidance, bridging of knowledge gaps, formulation of insightful questions, facilitation of communication with appropriate staff.
